This Agreement confirms the mutual understanding between Connections Community Services Society ("CCSS") and the Volunteer who signs below ("Volunteer"), regarding the Volunteer's role, responsibilities and obligations.

There will be a 3-month probationary period.

CCSS Commitments

CCSS agrees to:

• Provide adequate information, orientation and assistance to support the Volunteer in fulfilling his/her assigned responsibilities.
• Provide appropriate supervision and guidance during the Volunteer's assignment.
• Provide feedback regarding the Volunteer's performance and participation.
• Respect the Volunteer's skills, dignity, experience and individual needs, and make reasonable efforts to accommodate those needs where appropriate.
• Be receptive to feedback and suggestions from the Volunteer regarding ways in which we might better accomplish our respective tasks.
• Treat the Volunteer as an equal partner with society staff, jointly responsible for the accomplishment of the society's mission.

Volunteer Commitments

As a Volunteer with CCSS, I agree to:

• Perform my volunteer duties to the best of my ability.
• Adhere to CCSS rules and procedures, including record keeping requirements and confidentiality of agency and client information.
• Meet scheduled time and duty commitments or provide reasonable notice if I am unable to attend so that alternate arrangements may be made.
• Act as a cooperative member of the team supporting the mission and objectives of CCSS.

Confidentiality Agreement

I acknowledge and agree that any information obtained directly or indirectly through my volunteer work with CCSS shall be kept strictly confidential.

I will not discuss, disclose, share, copy, distribute, or otherwise disseminate confidential information outside of authorized employees and volunteers of CCSS unless:

• I am legally required to disclose the information; or
• The individual receiving services has provided authorized consent for release of the information.

I understand that this confidentiality obligation continues even after my volunteer relationship with CCSS ends.

Maintaining confidentiality is an essential component of any position with CCSS and a contractual obligation of the agency to funders. Breach of this agreement will be viewed as a breach of a condition of volunteering for the agency and is subject to disciplinary action up to and including dismissal.

By signing below, I confirm that I have read, understood and agree to comply with the terms and conditions outlined in this Volunteer Agreement and Confidentiality Agreement.
